School Coordinator

The School Coordinator will identify health plan members who may be eligible for and in need of school support services. The School Coordinator will work with the members/families along with their Case Manager to assess their needs and goals. The Coordinator will assist in developing an intensive care plan that addresses barriers to ensuring the member is successful at school. The School Coordinator will work with local school systems throughout the state and establish key contacts as it relates to school services, i.e. SEDNET program, Individualized Education Plan (IEPs), etc., to assist members/families navigate the system of care. The School Coordinator will be the Subject Matter Expert (SME) as it relates to obtaining behavioral health services and accommodations for our members. The Coordinator will help educate the member/family on the various programs and provide support for them as they learn to advocate for the services they need.

Essential Functions:

  • Initiate and develop relationships with the key contacts in the school systems throughout the state as it relates to Behavioral Health Services and Individualized Education Plan (IEPs)/504 Plans.
  • Coordinate with the designated Care Manager and member/family to develop a Care Plan that address the member’s behavioral health needs in school.
  • Initiate and develop relationships with both National Alliance on Mental Illness (NAMI) and the Federation of Families Advocacy Groups both locally and at the state level.
  • Act as the Subject Matter Expert (SME) on behavioral health services in the school for the integrated care management teams throughout the state.
  • Collaborate with the Member/Family, Case Manager, School System, and Providers (medical/behavioral) to ensure all are working towards accomplishing the member/families goals in a coordinated manner with good communication.
  • Work with the member/family to educate them on their rights as it relates to IEPs and 504 Plans and how to advocate for their needs with the school system.
  • Work with Multiagency Network for Students with Emotional/Behavioral Disabilities (SEDNET) and the School Resources Officers to support, not only our members/family’s needs, but the overall children’s behavioral health system of care.
  • Work with NAMI and the Federation of Families to help support their initiatives, as well as connect our members/families into their programs.
  • Collaborate with the Network and Provider Relations departments to ensure we have providers that are able to provide services in the school setting.
  • Troubleshoot any member/family issues as it relates to school services as necessary.
  • Performs other duties as assigned.
Additional Responsibilities:
    Candidate Education:
    • Required A Bachelor's Degree in Social Work (BSW), Psychology, Counseling, Rehabilitation, or other relevant field
    Candidate Experience:
    • Required 2 years of experience in Providing member support in a health services, behavioral health, managed care or other relevant field of work.
    • Required 1 year of experience in Working in or supporting members in a K-12 school setting.
    • Preferred Other experience with building and maintaining relationships with community leaders and partners.
    • Preferred Other knowledge of general school laws and policies as it relates to behavioral health services, (IEPs and 504 Plans).
